The Baby Naming Questionnaire

Such an exciting time in your life… considering a name for your baby! I am looking forward to talking with you on our scheduled 60-minute call. In preparation for this call I would like you to complete this questionnaire so I can better understand your needs and interests before we talk. I encourage you to think consciously about the many aspects of naming that perhaps you’ve taken for granted in the past. Feel free to add additional relevant comments at the end of the form. (All information is considered confidential.)

What is your baby’s due date?
Do you know if you are having a boy or a girl?
Who will make the final decision on your baby’s name?
List the first name and ages of any siblings.
What is the surname for the baby?
Will your child have a middle name, and if so, will that name be a family name… or one you have yet to choose?
Is the length of your child’s first name important to you? (In other words, do you prefer a name that cannot be shortened, or do you like the sound and/or look of multiple syllables?)
Do you feel you will allow or encourage your child to use a nickname as he or she is growing up?
Provide a list of at least ten girl’s and/or boy’s names that you are currently considering for your child. Include thoughts or comments on each choice.
Also provide a list of at least ten girl’s and/or boy’s names you very much dislike.
Is your family conservative and traditional in outlook or progressive and somewhat untraditional?
Do you have a preference in initials?
If you could choose your child’s future profession, what are three career areas that would get your vote?
Would you consider a name that is created specifically for your child, or would you prefer to choose a name that carries historical/cultural tradition?
Would you consider a traditional name that is spelled uniquely such as Nathanial or Steffani?
List the first names of 5 individuals who have been highly influential or inspirational in your life?
Do you wish your child’s name to have some association with your ethnic heritage by way of sight, sound or meaning?
Are there specific predispositions that appear to run in your family such as hyperactivity, addiction, weight management issues, depression or overall poor initiative?
Do you feel that you are a tough disciplinarian or someone who might have difficulty setting healthy limits and boundaries as your child grows up.
Will your child be raised with a stay-at-home parent or will he or she experience daycare at an early age? Is home schooling a possibility?
What do you feel will be your greatest asset as a parent… and your greatest challenge?
